How to get your COVID-19 Vaccination Receipt

I have a GREEN Ontario Health Card:

• Visit the Ontario Vaccination Service and use your health card to download your receipt: https://covid19.ontariohealth.ca/  OR
• Call the Ontario Vaccine Booking Line at 1-833-943-3900 for support accessing your vaccine receipt.  

I have a RED & WHITE Ontario Health Card

• Call the Ontario Vaccine Booking Line at 1-833-943-3900 for support getting your vaccine receipt

I do not have an Ontario Health Card

(Use this method if you have an out-of-province health card, or no health card at all)

• STEP 1: Get a COVID-ID
  Contact marissa.dean@porcupinehu.on.ca to get a COVID-ID.
• STEP 2: Get your receipt using your COVID-ID
  from the special COVID-ID website: https://covid19.ontariohealth.ca/receipt-identity OR
  Call the Ontario Vaccine Booking Line at 1-833-943-3900
