Step 1: Visit https://covid19.ontariohealth.ca/
Step 2: Scroll down on the homepage until you reach a section with a checkbox confirming that you have read, understood and accept the full terms of use. Click the checkbox, and then click the button that appears, reading: “Download your enhanced certificate or book your vaccine appointment”.
Step 3: Enter your personal information and click “Continue”.
Step 4: Under the service box “Proof of Vaccination”, click “Continue”.
Step 5: Click “Download your enhanced vaccine certificate”.
