Ininew Patient Services (IPS) will now require a signed electronic communications consent form in order to communicate personal health information by email. IPS must have an Electronic Communication Consent Form completed in order to communicate personal health information by email to patients. Please contact IPS toll free at 1-844-356-1178 or by email at referralsIPS@kingstonhsc.ca in order to request the form. The Consent Form can be emailed, faxed or mailed to your home. Consent Forms will also be available at your local health facility.
Please note that if you do not complete the form, IPS will continue providing all services to you by phone call and by regular mail.
